Melius, Schurr & Cardwell offers ob/gyn and other women's health care services at our Madison area office with the following medical insurance providers or networks:

• Health EOS (WPPN, HCN, AHC)

• Physicians Plus HMO
• Tricare
• Unity Choice*

• Commercial Insurances (e.g. Allstate, United American Insurance, Hartford, most Medicare secondary insurances that are not HMO affiliated). Basically, we accept a commercial insurance that allows you to see any physician or go to any medical facility.

Co-Payments are Due on Date of Service

Insurance cards must be presented at each appointment. Our practice accepts many forms of private health insurance. For any questions, please contact your insurance company directly or our business office at (608) 227-7007.

Financial Policy

Patient balances are due in full upon receipt of a current monthly statement.

* Choice plans are not conventional HMO plans and usually cover a lower percentage of costs.
