The Melius, Schurr, Cardwell & Raichle physicians and staff wish to congratulate you and are excited to help guide you through your pregnancy! Please thoroughly read all the documents included here as they may answer many questions you have or direct you to where answers can be found.
- Obstetrical Welcome Packet – contains information on the standard of care we provide for our obstetrical patients. You’ll find an overview of anticipated office visits, routine laboratory tests performed and ultrasounds you can expect to receive at our clinic.
- Obstetrical Welcome Packet Twins Supplement – contains information on the additional care routinely provided to our patients who are expecting multiples.
- Obstetrical Welcome Packet – Ultrasound Information – reviews typical ultrasound imaging during your pregnancy, their value and timing.
- Routine Obstetrical Laboratory Tests – discusses common laboratory tests that are completed throughout your pregnancy and reasons why they are required or recommended. Current Procedural Terminology (CPT) codes are also given for each procedure. You can supply your insurance company with these codes, in order to determine your coverage amounts and the total charge you may be responsible for.
- Fetal Movement Record – a form designed to help you monitor baby movements.
- Signs & Symptoms of Labor – discusses symptoms of true labor, determining if you are experiencing symptoms of false labor or areas of concern that require immediate evaluation. Information is also included on registering at Meriter Hospital for your delivery.
- Post Partum Discharge Instructions – advises what to do when you leave the hospital after giving birth and provides guidance on resuming normal activities.
